I always slate at the top, unless there is a request for none. I title my MP3's with my name and the job name. I also include contact info in the MP3 encoding. Ultimately, I want to make it as easy as possible on my (potential) clients.

After much twisting and turning on this issue, I've settled with a line at the end (recorded by someone else in several versions for various occasions) that just says... Colin Campbell - Custom audition. My thinking is if they get that far it will reinforce who they liked enough to get to that point. My guess is it is often not heard at all.
